Fear of having bowel movement

I have a two year old grandson, who from birth, has had issues with bowel movements.

He has always shown signs of constipation, however, his pediatrician doesn't feel it's an issue.

He has a poor diet (partly because he won't eat alot, and partly because he's fed more junk and nothing healthy).

According the the dr. his bowel movements are so hard, it's painful so he holds in it. When he does go, he will hide in a corner until he's finished, sometimes he cries.

He refuses to drink prune juice, eat corn or any other food that will help soften it.

His parents have had him on stool softeners for some time and recently stopped because they quit working.

Any suggestions on how to help this little guy??
