My son is the EXACT same way. The only thing that has helped him put on weight is Pediasure (with fiber). He was 17.5 pounds from about 15 months to about 21 months. He will be 2 in November. It's helped him put on weight so maybe it'll help your grandson as well.

Some people are just short. My daughter is 20 months old and she is under her ratio according to some chart her doctor shows me. She's short like her daddy, hes 5' 4". They constantly talk about her weight and health concerns but I don't worry about it. If your child is acting normal, then his body size is not going to stop him from his accomplishments. My daughter is little but climbs like she's a giant.
