Not sure whats going on..

Background: I am in my early 30s and a smoker (thats quitting!) I have PMDD and have been over weight most of my life despite being an active person. I was fit as a kid but when I hit puberty I got fat really fast.

The Problem: It all started last summer when I missed my period for three months. I thought for sure I was pregnant. I had all the signs and symptoms from nausea to cravings, fatigue, mood swings and so on. I took dozens of pregnancy tests and they were all negative, including one by my regular doctor who has no idea whats going on with me.

At the end of the three months I went into something that felt like either a miscarriage or labor. The cramps were excruciatingly severe and my normal routine of ibuproferin didn't even touch the pain. I could feel my uterus moving before the pain started and during the cramping as well. I honestly thought it might have been a baby moving at first before the pains set in. I had the back pains and everything and then wound up expelling an apple sized mass of purple tissue with white-ish threaded lines across it. After that came out (which was horrifying) I bled heavily and freely almost like I was urinating for a few hours. The ER doctors were vague at best saying that it might have been a fibroid, they weren't sure.

After this I had three normal periods (during which time I did lose small lemon to grape sized pieces of tissue but nothing massive) and then went right back to skipping them again for two months. Now that my period has finally started it's been incredibly heavy soaking overnight pads within about 3 hours and has lasted nearly two weeks of bright red blood with no signs of stopping. There have been a few small grape sized pieces of tissue and not to sound gross but also mucus type blood discharge which is also dark or bright red.

I have absolutely no idea whats going on. My regular doctor says that it could be polycystic ovarian syndrome but I've had ultrasounds and save for one or maybe two cysts, nothing is abnormal. I also don't fit the small breast category or "masculinization" category thats listed as symptoms. I've also always had very heavy periods that have lasted one to one and a half weeks and sometimes they were about 35 or so days apart but they were always there I never skipped a period and never lost those pieces before. Also since I hit my late 20s and early 30s my periods seemed to get less intense and more regular in the last couple of years until now. I've always had typical symptoms of PMDD, especially in the mood swing area.

I've also wondered if it could be due to some kind of infection but I'm not sure. Both of the weird periods after the missed ones started after I started on antibiotics for other reasons (abcessed tooth and bronchitis respectively.) My husband isn't circumsized either so I wondered if maybe I was having some form of bacterial vaginosis from that which I have had before because of it. I just don't know if it could cause such severe symptoms.

If you have any idea what possibly might be going on I could use the help!!!
