You've been through a very bad situation with your surgery, your chronic migraines, the pain medications which were prescribed and the situation with your physician. It adds to any stress you may have been feeling.
I do believe that you need to have another opinion on what has been done for you to date regarding medical care and also regarding the medications which were prescribed to you. I'm not an MD, but I do know that migraines that persist require somewhat more specialized treatment than might have been available to you previously. I believe there are a number of specialists in this area and they should be doing a complete evaluation to find out why you are having these migraines. Sometimes, migraines run in families and are often associated with depression.
You indicated that when you get anxious you start to shake and your speech becomes problematic. I have seen this happen in other people and this anxiety may best be handled by you by using the relaxation breathing technique that I may have recommended to you before. Give yourself a few minutes whenever you feel yourself becoming anxious. During this time, do the exercise, help yourself just to have a little time to calm down and then begin to slowly express what you wish to say. The thing to do is allow yourself time to step back from the anxiety and to collect your thoughts.
As I said, I think you need a 2nd opinion on a number of things and I hope that you do find someone who will work well and productively with you.
