question about Geodon Dr G?

bostonrocks1978 posted:
I have been diagnosed with bp and adhd. We recently discontinued my use of Geodon. We decided to do this because I wasn't taking it with food so he said that the amount getting into my system was negligible. Since then, I have been much more impulsive, restless, and been more forgetful-things that could be from the bp or the adhd. I know that I need to call my doctor but I would like to be able to know what I'm talking about when I talk to him.
The symptoms that have arisen could be attributed to the bp or the adhd. If the Geodon was barely in my system but the symptoms have gotten worse since coming off it would you suggest going back on the Geodon or trying something else targeted at the bp and/or the adhd?

Joseph F Goldberg, MD responded:
Dear Bostonrocks,

I wouldn't say Geodon is "barely in your system" if taken on an empty stomach. Rather, its absorption is about 50% less when taken on an empty stomach than with some food. If it is helpful, I typically advise a patient to continue it and try to take it with food to get the most absorption of the drug.

The decision about whether you should resume the Geodon (trying especially to take it with meals) versus try something else would best be a discussion to have with you doctor. The symptoms you mention aren't specific to any one diagnosis so I think it would be hard to know, without evaluating you, what they could represent. In patients with bipolar disorder, I myself prefer to focus on mood stabilizers (eg, lithium, Depakote) over antipsychotics as the mainstay drugs for long-term management.
