Dr. Goldberg, do you have any insight on sleep walking?

Something weird happened to me last night… I had a very vivid dream and in that dream I answered a phone and the call was for my roommate. So I got out of bed, walked out of my bedroom and knocked on his door at 6am and woke him up. He came to the door, I told him he had a call walked to the living room then I half way woke up and realized that I actually "acted" out the dream and felt sooo embarrassed and went back to bed. He said he could still hear me talking in my sleep… I know one other person on this board has similar issues with sleep walking, but does anyone else? And if you do how do you deal with it? Is there any way to prevent it? I'm scared I'm gonna do something dangerous while sleep walking… Of note.. I didn't start any new meds last night so I don't think it was med side effect.

On to another topic: I saw my pdoc yesterday and he upped my Topamax to 50mg bid and in 2 weeks I'll d/c Lamictal… Then he added Cymbalta 30mg because Trazadone 400mg wasn't enough to pull me out of my depression.. Once I'm out of it he'll d/c the Cymbalta… My pcp wouldn't give me anything for my chronic pain cuz they said everything would interact with my psych meds.. So I talked to my pdoc about it and he said it was B.S. and he rx'd Tramadol. So finally I'll have some relief from my chronic pain other than Motrin!!!

Also, we've come to the conclusion that I no longer have to take Antabuse on a daily basis… I have conquered the daily urge to drink alcohol, now I only have to take it if I know I'm going to be around it. Like at parties and so on… Which isn't too often, just every Sunday after my honey's softball games cuz the team gets together and parties after. So that's 2 pills less a day! Yay!! Little by little we're tapering off meds I think I can do without… I'm so happy about that cuz I worry about my kidneys with all the meds I'm on.

Dear Mozzergurl,

There isn't really any treatment for sleepwalking apart from figuring out if any medicines or drugs are causing it (eg, Ambien; alcohol/alcohol withdrawal) and then making sure the environment if safe so that a sleepwalker doesn't hurt themself. There are case reports that Topamax can can cause sleepwalking, and while it was once thought of as a possible treatment for mood disorders, the five negative placebo-controlled studies (coupled with reports that it can cause depression and drop IQ by 20 points) persuaded most doctors to think of it as possibly causing more harm than good...though it does have value to curb drinking behavior in alcoholics, and can cause weight loss.

Also, tramadol is a serotonergic drug similar to Cymbalta so hopefully your psychiatrist warned you about the possible interaction between the two that could cause serotonin syndrome and the potential lethality of mixing them, and what the symptoms of serotonin syndrome are to watch for.

Dr. Goldberg,

Thank you so much for letting me know that Cymbalta and Tramadol can cause serotonin syndrome...No my pdoc did not inform me of that.. I have a call in to him for that to see if he will give me a different pain med... I don't want to take the chance of that happening. My pharmacist said I would be fine as long as I was taking the tramadol only when absolutely necessary but I'm in severe pain almost on a daily basis and Motrin and extra strength tylenol don't touch it. I'm also going to talk to him about what you mentioned about Topamax...
Thank you so much for your advice!

Joseph F Goldberg, MD replied to mozzergurl's response:
Dear mozzergurl,

Lyrica can be dosed up to 600 mg/day and is commonly used to treat neuropathic pain as well as anxiety. It has no serotonergic properties making it compatible with serotonergic antidepressants such as Cymbalta. The symptoms of serotonin syndrome to watch for are jerking limb movements, sweating, teeth-chattering, and changes in heart rate and blood pressure. It is not a common, but a serious, possible drug interaction of multiple serotonergic drugs.

I was on topamax for a year and half a few years ago. It didn't cause weight loss, in fact i gained weight while on it. So be careful if your doc puts ya on it.

Thank you,

But I think I will have the weight loss effect because Seroquel increased my appetite and when I started taking Topamax it was curbed and I'm back to my regular diet.. I do still get hungry a couple hours after I take my seroquel but not as bad as before I started taking Topamax
