My husband is in mid-stage alzheimer disease and so far I have cared for him in our home. One big problem I am having is that he does not realize his need to shower, it has been 4 weeks now. All the advice I get is to start the water running, get out his towels, etc. for him. But this has not worked. I really think that the last time he did agree to shower, he forgot what to do after he stepped into the shower and started running the water, so now he refused to even try. I need some advice!

Sounds like you may need to move into the bed bath, or bathing him yourself stage. Try bringing a bucket of water to him, in an area where water mess isn't an issue and try to at least wash him slowly & carefully with a warm cloth. Or wash his face if he's very upset about the change. Small steps, no rushed action, and getting products like Rinse-free shampoo have solved our problem with my Mother-in-law. Hope that helps!
