1st appt with ne PM

Can anyone give me suggestions. I am going to see a new PM. I was dismissed from my other PM. I do not know what I did. I Called 5+ times wouldn't tell me. I sent for my med records and they stopped at my last visit.
How do I approach new doctor and what should I say so they will take me seriously. How much info should I give them. I am on meds still and have 2 the other pm gave me. I am going to get them filled right? I want to be honest also I have read on other sites to be careful to give out to much info. that may be better left unsaid.
The PM that dropped me I had been going to for 5 years. I thought we had a good doctor patient relationship. I was shocked and still am!!!
I owed some money because of the botox that my insurance didn't pay all of. She kept wanting to do it again but didn't because of the money. I am trying to pay it but I am on a fixed income. Could that be why? She also started me on different med. It may be that. I did get confused on the dirrections. I should have asked her but I didn't. So that it my fault.
Back to the question how should I approach the new doc so he won't see me in a bad light.
