Delayed Ejaculation-relating to a medical condition

I would love some advice or help on this topic. I have been with my partner for over 4 years now. He is 26 and I'm 23. We have the most fantastic sex life but he is unable to ejaculate through intercourse, however he can through oral or masturbation. It can take anywhere between 1 to 3 hours for him to achieve orgasm through either of the methods above. Sometimes he does not achieve orgasm at all. At this point I feel it's important to mention his medical condition, he has has a urostomy or stoma since the age of 11. This has never been an issue in our sex life. He says he enjoys our sex life fully and that there is nothing that I do wrong, however I feel guilty that he can't achieve orgasm or that it takes him so long, when he can make me orgasm numerous times. He is comfortable disscusing the topic with me, but will not speak to a doctor and I do not want to apply any pressure as I know this is a sensitive topic for him. If there is anyone out there with any knowledge that could help I would be incredibly grateful as we would like to have children through conventional methods if possible, and I would also like to give him the satisfaction he gives me.
