Back in 2006 I had surgery done to remove a dermoid cyst. This required them to remove both my right ovary and fallopian tube. I had a baby in 2009. I have noticed recently that I have been getting these cramp like sharp pains in my lower right abdomen. These always occur the week before I start my period. They kind of remind me of period like cramps however I find it odd that I would be getting this pain on the right side of my body where I dont have an ovary or tube. I never get these cramps on the left side of my body. Any suggestions as to what might be casuing these. Possible just scar tissue from the surgery or a friend mentioned something with my appendix maybe??

Jane Harrison Hohner, RN, RNP responded:
Dear Glaze: Both surmises have the potential to be correct. One can develop adhesions (filmy bands of scar tissue) in an area where a surgery was done. What is unclear would be why the pain only happens a week prior to menstruation. Some women with endometriosis can have cramps which onset well before menstruation, yet this seems unlikely in your case as the pain is so focused in only one area..

One's appendix, small bowel, and ascending large colon are in the lower right abdomen. Again it would be hard to know why these would act up only before your period.

If you were my patient I would ask you to come in for a pelvic exam and perhaps an ultrasound when the pain was present. That would give the best change to see if a specific body site was involved. Unfortunately, adhesions do not show up on ultrasound or X-ray, but at least appendix/bowel/ovarian remnant syndrome could be assessed. So do go back to your GYN or clinic when you know the pain will be happening.

Yours,
Jane
