Severe Anal Pain during menstration

I have always had extremely heavy/painful periods and was told it was caused by ovarian cysts (I started at age 10). After I had my daughter (jan 09, age 22) I started experiencing SEVERE, knock-you-to-your-knees pain during my period. It hurts to sit or go to the bathroom (bm or urinate). I am now 25 and the pain continues to get worse. I know that my cervix tore during labor and I hemorrhaged. I also bled for 7 months (continually) after I had my daughter. Not one doctor that I have seen can give me an explanation for any of my symptoms. I was told to deal with it, but its to the point it's interfering with everyday life. I have been on Ortho-Cyclin, Ovcon 50, Seasonique, and Depo among other BC.. it lightens the bleeding somewhat but does nothing as far as helping pain. Please can you give me an idea of what I could be dealing with?

Dear An: One of the most common explanations for rectal/anal pain associated with menstruation is endometriosis which has infiltrated the bowel. Endometriosis is also famous for prompting the most severe and prolonged pelvic pain during menstruation. The implants of endometriosis tissue can attach themselves to the bowel, bladder, and ovaries. They can even invade the muscular walls of the uterus ("adenomyosis").

Often birth control pills (especially the continuous type) and DepoProvera (when it stops menstruation) are helpful in reducing the pain of endometriosis. Thus, endometriosis may not be the sole culprit, but it is worth ruling out.

I would urge you to get a second opinion, be sure to ask about endometriosis. If you have a history of endometriosis in your blood family be sure to mention that as well. It used to be assumed that teen aged women would not have endo, but more recent studies have demonstrated that endo can be present in younger women as well.
