Hi Jane it;s me again I am troubled with some small problems I am almost finished with my UTI 2nd round of medicine and still no relief on-top of that I have discharge and vaginal swelling the discharge is white and it burns when I saw my GYN he said I probably would get a yeast infection but I have new symptoms from the UTI my sides are now hurting and I am having some mild cramping not sure what to do he did say that if after three to four days and no better to call but they are not open monday and then there is the whole pain meds thing with all this going on I am in a pickle I have no pain meds left and tynol or IBU's don't help at all I don't want to go to the ER because since I have a pain contract with my GYN he can only give me pain meds but at this point he said it might be really hard to give me anymore without a diagnoses so what to do I just want this to end I fear that I am going to have a withdrawl and something bad will happen and then what

Dear smjpain: If your UTI is not improving, and you are having side pain/back pain/cramping you may need to see the ER and/or notify your GYN's office. This would be to rule out a kidney infection.

I hear what you are saying about running out of pain meds. Not only do you not have pain meds when you need them, but there is the concern about an abrupt withdrawal. I have forgotten if you were prescribed Percodan (oxycodone) or Vicodan (hydrocodone) for management of your pain so this will be a general answer. According to what I could find out, withdrawal starts after about 24 hours off, and symptoms can feel like the worst case of flu--ever. Symptoms abate as soon as the drug is reintroduced.

I don't know how many per day your were allowed on your pain med contract. That here was a contract speaks to how long the pain medication has been needed. Once you get another prescription from your GYN it may be time to open the same discussion with your GYN that you are sharing with me--your fears that they may stop prescribing, that you might have bad symptoms, etc. I would bet money that they will be glad you were open about these fears.

Hi Jane I was taking Lortab 5/500 twice a day and when I saw my GYN on wed he said that he didn't think he could give me anymore I ran out yesturday and right now I am experiencing Dyria and I feel scared I want to call my GYN but I don't want to go to the ER because of this problem I know they know me and my situation and they will not give me anything unless they get ahold of my GYN I am going to call his office right away in the morning and see what he recomends for everything I also forgot to mention my right breast that was infected a month ago is swollen and very sore today and also has a scally feeling on the nipple. Jane I just don't know what to do

Dear smjpain: Taking two of the Lortab 5/500 is like taking two Vicodan 5 mg and two 500mg Tylenols per day. That is a relief to hear as I was worried you were taking many more or a higher dose (Vicodans come in 2.5, 5, 7.5 & 10 mg doses). Still it would be good to get off regular use so you can get better pain relief when you have an acute increase in pain (eg dental pain, surgical pain, etc).

The right breast needs to be evaluated with an exam to see if the swelling/tenderness is related to the prior infection or a hormonal reason (do you still have one ovary?). Also, the dysuria should be rechecked to see if that pain with urination is from an infection or urethral issues.

Hope you were able to get an appointment with your GYN promptly to get some answers to your three concerns.

Jane I couldn't get ahold of my GYN today but I left a message to call me in the morning because I went to see my regular doctor for my breast because It was starting to make me sick and the pain is bad anyways she said it is infected again and swelling up also she looked at my hands because I have really bad ezma anyways they blistered and now are opened many cuts she said I HAVE a staff infection and can't go to work I didn't think to have her do a yeast check and UA but should have now I might have to go to the ER tonight because of the pain and she told me if my temp gets to 100 I need to no matter what I am scared she said the staff might be in breast and is very worried she gave me doxyclyine 100mg 2 a day for five days if my hands are not better by thursday I have to call right away I don't want this anymore I hate the ER they mostly all now me and it is almost like they don't listen to me.

Dear smjpain: If you do have a UTI, the doxycyclin 100 mg twice a day is an accepted treatment--if your UTI bacteria is sensitive to doxycyclin. You MAY get a worsened yeast infection, but treating the breast infection is paramount. If you have successfully used over the counter yeast medications you could consider using one as a preventative in light of the new antibiotic exposure.

As for the ER, it can be true that "frequent fliers" (frequent visitors to the ER) feel like they are not being listened to. Your primary care and GYN MDs sound like they take you very seriously, and treat you with respect/compassion. Yet if those MDs direct you to go to the ER you need to adhere to their instructions. It is also OK to say to a MD/nurse/tech in a non-confrontational way "I don't feel like you are listening to me." It might give them cause to re-examine their tone of voice, body language, or other communications.
