Jane I am writting again I will not going into major details about my weekend because I did in another post well friday GYN said I have a yeast infection yesturday I had to go to the ER twice for back pain and a UTI so today I had to go back in again because I couldn't pee and hadn't gone since 3am anyways I was there for five hours I saw a PA Lindsey was her name I have seen her alot in the ER just a few weeks ago for a yeast infection she drained my bladder did a vaginal exam no speculum because I was in so much pain and very very red and swollen just her touching me I cried so bad she said there was alot of discharge and a very strong odor she told me that I need to get into see my GYN in the morning no matter what she gave me some pain meds and strict orders to follow up with my GYN and if I still am not peeing or have a fever yet tonight I am supost to go back to the ER. What is going on I feel sick very sick my Vagina hurts so bad I just want to rip it off. Oh they gave Macrodaten 2a day for seven days but also said they should check to see if it is going to work on monday otherwise IV meds will be needed. and as for the yeast infection she said I need something stronger than Diflucan. Any thoughts on what my body is telling me also I am wondering how to make it less painful during the exam he is always very gentle and if it hurts he stops or the nurse holds my hand also I know I have to see my GYN because of me also not being able to have sex still he told me to try over the weekend and we did on friday and I started bleeding and he hardly got in only about an in and he didn't have a condom on. Called my GYN today and he can't see me till thursday but I know the UTI is getting worse and the Yeast infection I even tryed getting in with my regular Doctor no luck should I go back to the ER or notThanks for your Reply!

Dear smjpain: The ER give you medication for a probable UTI, and hopefully then send in a catheterized urine sample (from draining your bladder) for a urine culture and sensitivity. Those results, confirming that the Macrodantin should work, are usually available within 72 hours. If you are still having urinary problems (and your regular MD or GYN cannot see you) then yes, you should go back to the ER.

Quite honestly I am perplexed about your intractable yeast infection. A yeast CULTURE can identify what subtype of yeast is present. Am I remembering correctly that all vaginal cultures done about two weeks ago were negative? Perhaps a yeast culture was not done.... If you do have a non-Albicans type of yeast the usual yeast medications may not work as well. Under those circumstances a GYN may try a nontypical type of treatment (eg Gentian violet, or even boric acid capsules). However, in your specific case with the high degree of inflammation, your GYN may be hesitant. It is also possible to have fluconazole resistant types of yeast. There are over 2,000 citations at the National Library of Medicine site on this.

If you have drug resistant yeast, your GYN may need to do a phone consult with an infectious disease specialist (usually found at a University hospital or teaching hospital). Thus you should definitely follow up with your GYN about the vaginal irritation.
