I have just started trying to have sex I am 28yrs. old but so far my BF can only go about an inch before I start bleeding and screaming it burns so bad down there and I can't even masterbate myself even him touching me hurts I had a Total HISS when I was 27 for Endometriosis and this was my first time I have already had my Hymen broken from a speculum exam anyways went to my GYN today and he did a spec and took some samples looked under a micro-scope and said I have B.V Bacterial Vaginosis and you can get that and not have any symptoms at all and one symptom is bleeding after sex. I also suffer from Lichens sclorosis and vulvadina and vagnisms. So I would make an apt. with your GYN ASAP P.S B.V is not a sexually transmitted disease

Dear An: There are two basic culprits that can cause bleeding after sex: problems with the cervix and things that create bleeding from the uterine lining. Bleeding coming from the cervix could come from a cervical lesion---if one has had a recent normal PAP smear this is unlikely. An infection of the cervix (eg "cervicitis" from Chlamydia) can make the cervix more "friable" (easier to bleed). This would be unlikely if you are in a monogamous relationship!

In some women there is a normal enlargement of the area of glandular type tissue. These women can have bleeding when the cervix is sampled for a PAP smear. A polyp coming from the cervical canal may bleed only when the cervix is touched. Endocervical polyps of this type may be readily seen during a speculum exam. I would really doubt any of these are your culprits since this was a first time episode.

If the uterine lining is easily destabilized, having sex can prompt spotting or breakthrough bleeding. Some women will have this type of spotting if sex occurs during ovulation or right around the time of menstruation. My best GUESS is this is the casue of your bleeding. Women using hormonal forms of birth control may also have less stable uterine linings.

.Endometrial polyps or uterine fibroids can create a focus for unstable uterine lining. If a woman has a history of missed periods, her uterine lining may be very thickened. In that situation spotting can represent small amounts of the lining being shed---just off the top layer.

Given the multiple causes of bleeding after sex, one should go see a GYN if the spotting persists or is recurrent.

Yours,
Jane
