Dear slw: Currently you are on birth control pills, but are still troubled with poor cycle control/breakthrough bleeding (BTB) accompanied by severe cramping. This pain is worse on the left side and is only present when the bleeding occurs. Hope I am tracking you correctly. There are also premenstrual symptoms (severe fatigue, insomnia, headaches, and GI problems such as bloating, GERD, and nausea). Of all the medications you have tried, birth control pills have helped somewhat, as has the Naproxen.
Some medical conditions which MIGHT be linked to your symptoms could include:
1. Adenomyosis--this is endometriosis of the muscular wall of the uterus. It has been linked with uterine pain and erratic bleeding. It is very difficult to diagnose with ultrasound, an MRI may be needed. It is usually diagnosed by a pathologist on a hysterectomy specimen.
2. Endometriosis of the descending colon/bowel--this would provoke the left sided pain, but not the erratic bleeding patterns.
3. Bowel based problems--these can prompt bloating, gut pain and may be seen in conjunction with upper GI issues. The second most common cause of GYN pain (once GYN causes have been ruled out) is the bowel. Diverticulosis, IBS, and other bowel problems can present with symptoms similar to pelvic infections. Yet bowel conditions would not trigger erratic bleeding.
Honestly, it might be that there are coincidental diagnoses rather than one common cause. You might end up working with your GYN on the bleeding issues and a GI MD on the upper and lower GI problems. Decisions about pain relief usually hinge on having a correct diagnosis.
I am really sorry that you have had to endure this loss of your previous quality of life. It would be amazing if you were not experiencing depression on account of this undeserved loss at such an early age.
