    Cholesterol and Apolipoproteins test - Please decipher

    I have had a a confusing month with cholesterol tests. Had my annual and cholesterol came back at a whopping 315, ldl 215 hdl 77 and tri 87. Also was pre-diabetic. For a week, I completely changed my diet to low fat, low cholesterol and began exercising. Went to a metobolic place for suggestions and also asked for a re test since this went up from 240 last year total to 315 this year. The retest came back at 230, ldl 133, hdl 87 and tri 53. Not sure how this happened so quickly. Actually still think the lab could be at fault but could get no explanation from either my doctor or the specialist. Also had a apoliprotein (sp?) a b test and the results were a 182 and b 94. Have no idea what these results can tell me and would love an in depth explanation. They just told me that the b should be below 80 if I had heart issues, which I don't. Not sure if my current cholesterol is an issue since I have the high hdl or if I should still be trying to lower it with meds if diet and exercise don't work. I am a relative thin person, don't smoke and actually thought I was eating pretty healthy before this. I have cut out cream in my coffee, red meat, which I didn't eat a whole lot of anyway and chocolate, ice cream and processed food such as cookies etc. Also stopped eggs. Hoping this will help.     It is the ratio of Apo-B to Apo-A1 that is computed to determine your risk of heart disease. The normal values are 0.5 to 1.0.

    In your case the Apo-B / Apo-A1 ratio is 94/182 which is 0.52, and excellent value. Values above 1.0 indicate heart disease risk.

    The normal range for Apo-A1 and Apo-B are:

    Apo-A1: ADULT : 115 - 210 MALE : 110 - 205 FEMALE : 125 - 215 mg/dL

    Apo-B: ADULT : 55 - 135 MALE : 55 - 140 FEMALE : 55 - 125 mg/dL

    In your case these are in the normal range too.

    Your cholesterol levels look good too.

    You food habits are fine. Just ensure that you consume citrus fruits in order to get a lot of vitamin c, which will keep your circulatory system elastic and healthy.

    "Also stopped eggs" Actually eggs are a good food.Just limit them to 1 or 2 a day.

    I suspect that the first reading was an outlier. Maybe a lab test. I had one that was out of line with my others. It was done about 10 days after shoulder surgery. Maybe the pain meds?

    A number of doctors feel that extended cholesterol testing give a better picture than the basic cholesterol test. However, there are several different types of extended. And each reports different data.

    ApoA relates to the number of HDL particles. ApoB relates to the number of LDL particles.

    Berkly Heart lab indicates that for ApoB normal is less than 60. Intermediate risk is 60 to120 or 60 to 100 depending on the test. And above 120 or 100 At Risk.

    I don't know what kind of "metabolic specialist" that you saw. But I am surprised that a doctor gave you an extended cholesterol test that does not understand what it means.

    Also with pre-diabetes you might have insulin resistance. And that also increases the risk of future heart problems.

    If it was me I would see a lipid specialist, typically a cardiologist, to go over all of this and give you his recommendations.
