My husband age 55 who is oxygen dependent due to Alpha One deficiency has developed significant dependant edema 2-3 pedal to knee. He currently takes losartan/hctz 100/25 dly, metoprolol 50mg dly, furosemide 80mg dly along with his lung meds including a wkly infusion of aralast NP. He is also on an alternating dose of coumadin 4 and 6mg due to pulmonary emboli and atrial fib. last December. His lungs are clear, no crackles or wheezes. His appetite has decreased in the past 2 weeks and he has continued shortness of breath though it is difficult to say if this is heart related or due to his severe COPD. He will see his regular doctor tomorrow and is seen regularly by a pulmonoligist for the alpha one. What are your thoughts.

James Beckerman, MD, FACC responded:
It's very hard to say, based upon his multiple medical issues (Alpha 1 anti trypsin deficiency combined with AF and pulmonary emboli). I'm hopeful that with an exam and possibly with labs or other testing, his doctors can help figure out what's going on - I wouldn't want to make an opinion without all the information. Take care.

I appreciate you responding. He is followed closely by our family MD and was started on Torsemide yesterday. Continues to have significant edema but pulse oxys are acceptable and lungs remain clear but diminished. You are correct about the multiple medical issues and as a nurse myself sometimes a little knowledge is worse than no knowledge. We will continue to take it one day at a time. Thanks.
