4 yr old with Hepatitis A even though vaccinated

My 4 year old daughter was just diagnosed with Hepatitis A. My question is how is this possible when she had all her Vaccinations against Hepatitis A, and there have been no known outbreaks in the area we live in?

If she had the hepatitis A vaccine less than 6 months ago, she may not be immune.
