Extreme and Sour Foot Odor

My husband has a foot odor issue that we cannot clear up. we've tried odor eaters, shoe inserts, powders and sprays but nothing helps. We buy good quality shoes and he is wearing only cotton socks.

Sometimes the odor is detectible even while he still has his shoes on, when he takes his shoes off the smell is sour and very strong. What can we do to help the issue?
