I just signed my 1st grader up for camp and they needed proof that her shots were up to date.

I had to do that last week for camp. Our family doctor provides a shot record and keeps track of when we need vax's.

Every year after their birhtdays we ahve to provide a health report for daycare and I just did it for James's school registration. I have a yellow card given to me at their first pedi appointment. I guard them like fort knox. The pedi just photcopies it and attaches her to the Dept. of Ed's health report form.
