Why did neutering my 2yrs old Shih Tzu cause his death?

I lost my Shih Tzu a few days ago, and feel extremely guilty about taking him in to be neutered. He had a clean bill of health from his vet, all of his vaccines were current. The vet that performed the surgery said that the surgery itself went fine, it only took 40mins. Then he said my pup went into cardiac arrest after the surgery was over. I don't understand, and I need some type of closure. The vet said he can't explain what happened. It may have been an allergic reaction to the anesthetic. I don't understand, why after 40mins of being under, my pup all of a sudden gets an allergic reaction? I had my vet from another hospital perform a necropsy, and she told me EVERYTHING was normal with my pup, there were no abnormalities. I don't understand how this could have happened. I'm extremely heartbroken, all I want are some answers.

Please let me know if you've ever gotten answers to your terrible tragedy. We have just experienced the same exact thing and we are confused and distraught.
