I don't get it either. Self-esteem has a lot to do with the ability to get and maintain an erection. A prescription is required but a doctor cannot prescribe esteem. If the prescription substance is not taken internally it must either be an injectable or a topical gel or cream. The Germans have perfected an inexpensive gel that works instantly and much longer lasting than Viagra, but the US pharma companies will not permit or democratically elected representatives to approve it for use in the US. That leaves injectible drugs that are available with a prescription, work quickly and sustain an erection for a long time - sometimes so long emergency room treatment is necessary. Perhaps An_208193 means that after taking that stuff his "esteem" is strong enough to allow him to stop speeding locomotives and jump tall buildings in a single bound.
