Ketoconazole, pill or cream?

I was prescribed this medication along with hydrocortisone because my Dr. believes the area on my chest/breast is fungal. I get itchy/scabby nipples and areolas...and the skin on my chest and top of my breast has little patches of scab..

In addition to this problem, I also believe I have hs because I have hard boils everywhere (inner thigh, groin, ALOT of little pockets under my breast)....but I'm wondering if it can also be fungal because HS patients don't have little pockets of pus everywhere in addition to the hard boils do they?

My main question is should I be taking the pill instead of the cream ketoconazole, a pill seems like it would treat every area quicker. I need to call my dr. to see if it's okay to apply these creams to the hs areas.

I also want to add that I have PCOS and just started on metformin, I'm also insulin resistant
