Stroke with Bacterial Meningitis (Very urgent)

Dear Doctor,

My mother name is Gulzar (Age - 48 yrs), four months ago she was suffering from head ache, vomiting and fever for 15 days at that time we were checking with doctors, finally after 15 days we came to know that my mother is suffering from Bacterial meningitis, she was given the medication for that. She was improving, but now for the past one week she is not able to speak, move her hands or eat.

When we checked with the doctors, they are saying some small strokes have occured in cerebellar region and there is no cure for that, and they are saying rehabilitation will not be useful for her, so they don't her to go for that. Is there any possibility she can be cured?

She is not suffering of drug reisitance bacteria they are saying, they are also saying the steroids of DEXA are not making any difference or improving her health? and now they are saying all the results are negative except, Bacterial meniningitis which is 80% positive, Please help me.. and my mother,

Please reply me.. or give me ur email id, i will mail you all the reports, as the doctors are not able to diagnose what the problem is.

I am sorry to hear about your mother . Can you answer a few question for me?
1. Where does your mother live. This will give me an idea of the resources available to her.

2. Do you know the name of thebacteria that caused the meningitis?

3. How long has it been since they started tretament with antibiotics and is she still on the medicine?

Thanks for replying,

1) My mother lives in India (Near Chennai)
2) I don't know eaxctly, but in the reports they have mentioned is Myco Bacteria
3) Its been 4 months since she is getting anti biotics, yes she is still on medicine..

Dear Haseeb,

I am sorry for the delay in replying to your e.mail. Your mother has been diagnosed to have TB meningitis based on her history, physical examination, CSF and MRI brain findings. In her case, the culture from the CSF has not grown any TB bacilli. In most with TB meningitis this is also the case. The treatment for TB meningitis is usually started based on the science and symptoms, CSF and scanning reports and that is what we have done for your mother. Some percentage of people has TB meningitis do not respond adequately to medications. When the patient does not respond, we mean that they continue to have complications like hydrocephalous and stroke like your mother has. As the culture has not shown any resistant organism, we can not say that she has drug resistant TB but we have considered that issue also. Her present condition is consequence of TB meningitis and we have not find any other diagnosis so far. This diagnosis has also been confirmed by the neurologist. There was suggestion of brain biopsy to get 100% proof of TB but it is not possible in her present condition. As I mentioned earlier, a certain percentage of people with TB meningitis do not respond to therapy and we can only give supportive care as in the case of your mother.

The PMR doctors wish to give a home programme rather than admit under them because in their experience they find that patient with TB meningitis do not benefit by admission under PMR. We understand your distress about your mother's condition but we will be unable to keep in hospital after a period of time. She is right now in a chronic stable consition and she will need to be under follow up on therapy. You have asked about night sweats and her cold hands. I have explained the reason for this to your sister. The problem is more of the type of disease than the therapy. Unfortunately some patients with TB meningitis do not recover as expected.

Thank you for sending such a comprehensive report. It appears you mother's doctors have done all the right things. It also seems that she is too sick to benefit from a rehabilitation hsopital at this point. We rarely see TB Meningitis in the United States. I saw quite a few cases in children coming from near the Mexico border when I first went into practice. Unfortunately it is a very serious disease that is very hard to treat. The doctor's note suggests that they have done all the right things and that inspite of those measures, the disease has the upperhand.

Here is a link to an excellnt review of TB Meningitis. It won't make you feel any better about this unfortunate situation, but it should help you see that she is getting the care she needs.
