Nausea then temporary difficulty of speech

I have a grandson who is 20 years old and for last 5 years or so every few months he has an episode where he has nausea for 10-15 seconds then loses ability to speak but still has full thinking ability. This lasts for 2 minutes or so. Same day usually has 2-4 similar episodes, then nothing for several months. Has had MRI/brain scans that show no seizure activity and his doctor is stumped. Any thoughts on which direction we should go? Thank you
