    WH-380-E is the form you will use for yourself. Make sure if you want to use your FMLA at work..you have your doctors put in INTERMITTENT LEAVE. This will cover you for all doc's appointments, surgery, leaving early, if you come in a little later...

    But just a general fyi..being there in the morning is more important than leaving early..don't know why..but as long as you show up ontime, leaving early doesn't seem to bother HR as much.
